Security Health Plan and the Wood County Health Department believe in improving the health, lifestyles and overall well-being of the communities we serve. As part of our commitment, we are supporting childcare center and school gardens to help children increase vegetable consumption and develop healthy habits. HERE is a document that will help prepare you for filling out the application below.
Sprouting Healthy Kids Grant Application
Eligibility Requirements: Applicant's must be a certified or licensed childcare center, or a nonprofit K-12 school or 501(c)(3) AND be in Security Health Plan’s Service Area. Click HERE to view counties within the service area:
IMPORTANT: This application needs to be completed in one sitting, and will take about 20-30 minutes depending on your responses.
